What happened?
Nature Medicine reported exploratory analyses from a placebo-controlled randomized phase 2b trial of Descartes-08, an autologous RNA-encoded anti-BCMA CAR-T therapy for generalized myasthenia gravis. Durable clinical efficacy was observed in 10 of 15 treated patients in the reported analysis, without lymphodepleting chemotherapy.
Why does it matter?
Conventional CAR-T can require intensive conditioning and inpatient monitoring. A transient mRNA CAR-T approach aims to retune pathogenic immunity while avoiding permanent cell engineering and some of the logistical barriers of oncology-style CAR-T.
How to interpret the evidence
The trial is small and the field is still early. Larger confirmatory studies are needed before this can be considered a routine autoimmune treatment.
What remains uncertain?
This is not evidence that CAR-T is ready for common autoimmune diseases or that standard myasthenia treatments should be stopped.
