What happened?
In the phase 3 ALLEGORY trial, 303 adults with active systemic lupus erythematosus were randomized to obinutuzumab or placebo in addition to standard therapy. The SRI-4 response at week 52 was reported in 76.7% of the obinutuzumab group versus 53.5% with placebo.
Why does it matter?
Lupus treatment is moving beyond broad immunosuppression toward more selective immune pathways. A positive large phase 3 B-cell depletion trial is therefore an important development for systemic disease.
How to interpret the evidence
The benefit has to be balanced against infection risk, vaccination planning and the wider safety considerations of potent B-cell depletion. Individual disease phenotype still matters.
What remains uncertain?
It does not mean every person with lupus needs obinutuzumab, and it does not remove the need for background therapy or specialist monitoring.
