What happened?
On 17 August 2026, AstraZeneca and Daiichi Sankyo reported positive high-level results from DESTINY-Lung04, a global randomized open-label Phase III trial in 454 patients with unresectable, locally advanced or metastatic non-squamous NSCLC harbouring a HER2 exon 19 or 20 mutation. Enhertu (trastuzumab deruxtecan) produced a statistically significant and clinically meaningful improvement in progression-free survival by blinded independent central review versus platinum-pemetrexed chemotherapy plus pembrolizumab. The trial continues for secondary endpoints including overall survival.
Why does it matter?
HER2 mutations occur in a small but actionable subgroup of NSCLC. Trastuzumab deruxtecan is already used after prior treatment in HER2-mutant metastatic NSCLC, so a positive first-line Phase III result could move HER2-directed therapy earlier in the treatment pathway if the full dataset, regulators and future guidelines support that use.
