What happened?
In the phase 2 EMBRAZE study, participants receiving tirzepatide plus apitegromab lost less lean mass than participants receiving tirzepatide plus placebo. The myostatin-targeting antibody was generally well tolerated in the trial.
Why does it matter?
As obesity pharmacotherapy becomes more potent, attention is shifting from kilograms alone to body composition, strength and long-term function. Preserving lean tissue may be particularly important in older or frail patients.
Healthway perspective
This was a proof-of-concept phase 2 study. Preserving lean mass on a scan does not automatically prove better strength, mobility or long-term clinical outcomes.
What this does not mean
It does not mean everyone taking tirzepatide needs a muscle-preserving biologic or that resistance exercise and adequate nutrition become unnecessary.
